29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a) On or about July 10, 2012, employees were exposed to falls in excess of 6 feet while installing a HVAC system on a flat roof of a commercial building.      Abatement Note:  Abatement certification and documentation is required for this item (see enclosed "Sample Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:    a) On or about July 10, 2012, employees were exposed to a fall in excess of 6 feet while using an extension ladder that did not extend 3 feet above the parapet wall and was not secured..
