The employer did not provide medical evaluations for employees who were provided respirators, on or about 7/13/12.    b) East end of warehouse: Employees were provided a 3M 6200 half mask respirators to use when mixing additive masterbatches. The employer did not provide medical evaluations for employees who were provided respirators, on or about 7/13/12.
