29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    a)  ARIZTA / LaBrea Bakery, Manufacturing Plant:  The employer did not develop specific procedural steps for all types and forms of hazardous energy such as electrical, mechanical, hydraulic, pneumatic, chemical, thermal, etc., associated with equipment which includes but is not limited to: De la ballina metal detector, Miwe - Horno, Rheon, San Cassiano, De-Palletizer, Wexxar, LeMatic Slicers, Pattyn to effectively control them, occurred on or about 07/12/2012.

29 CFR 1910.147(f)(1):     When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed:     a)  ARIZTA / LaBrea Bakery, Manufacturing Plant:  The employer did not follow the sequence of actions to temporarily remove LO/TO devices from machinery to test or position the machine, equipment or component.  The machines are re-energized to accomplish tasks such as diagnostic testing, maintenance troubleshooting, or machine positioning set-up, occurred on or about 07/12/2012.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    a)  ARIZTA / LaBrea Bakery, Manufacturing Plant:  The La Ballerina machine control panel had an unused opening a missing knock out cover where the on/off discharge button used to be located, observed on or about 07/12/2012.
