29 CFR 1910.23(e)(1):     The railing did not have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level and the intermediate rail was not approximately halfway between the top rail and the floor, platform, runway, or ramp:    Located at 65 Shun Pike, Johnston, RI, Power Plant: On or about 06/15/2012 employees were exposed to fall hazards from the Cotterman Work Platform in that the hand-railings were not adjusted to a height of 42 inches.    Abatement Certification and Documentation is Required for this Item

29 CFR 1904.7(b)(3):     How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known.    Located at Providence Power LLC, 65 Shun Pike, Johnston, RI: On or about 06/15/2012 the employer did not correctly record the number of days away in Column (K) in the following instance:    (a) On or about 06/15/2012, Case number 1-Dec, Mechanic - An employee sustained a head injury from a fall from an elevated work-platform resulting in days away whereby the employer recorded "continuing" in Column (K) and should have reflected the number of days as specified by a doctor or other licensed health care professional.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    Located at Providence Power LLC, 65 Shun Pike, Johnston, RI: On or about 06/15/2012 the employer did not record the following workplace injury within seven (7) calendar days whereby the injury resulted in days away:    (a) On or about 06/15/2012, Mechanic - An employee fell from an elevated work-platform resulting in days away due to a concussion whereby the employer did not record the injury until approximately 07/23/2012.
