The orange extension cord was wired directly into the temporary power panel and the cord was not used in accordance with instructions included in the listing, labeling, or certification.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which were used by employees did not have approved ground-fault circuit interrupters for personnel protection: Employees were exposed to electrical shock and burns because an extension cord was used to provide electrical power from the control panel to a hand held grinder because the circuit or the cord were not protected ba a ground-fault circuit interrupter (GFCI) nor a assured grounding program.
