29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a- St. John the Greek Catholic Church, corner of Broad St. & Division St., Perth Amboy, NJ - Employees engaged in roofing activities, approximately 18 ft. from the eave to the concrete ground below, were not using any fall protection system. Condition noted on or about 7/14/12.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a- St. John The Greek Catholic School, Perth Amboy, NJ - The Werner 32 ft. fiberglass ladder used to access the roof was not extended 3 ft. past the eave of the roof. Employees were exposed to fall hazards of approximately 18 ft. to the concrete ground below. Condition noted on or about 7/14/12.
