29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) On or about July 5, 2012, employees were not protected from fall hazards in that the ladder used to access the roof was not extended at least three feet above the eaves at a house located at 1432 N. Murry Blvd. in Colorado Springs, Colorado.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    (a)  On or about July 5, 2012, employees were not protected from fall hazards while reroofing a house whose eaves were 9.6 feet above the ground at 1432 N. Murry Blvd. in Colorado Springs, Colorado.    Candalario Lopez was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 312143191, Citation 1, Item 3, issued September 11, 2009, with respect to a workplace located at 11569 Terrawood Ct. Parker, CO, 80134.  This citation became a final order on October 8, 2009.
