29 CFR 1926.403(i)(2)(iii): Entrances to rooms and other guarded locations containing exposed live parts operating at 600 volts, nominal, or less were not marked with conspicuous warning signs forbidding unqualified persons to enter: (a) Location - 1st floor switchgear room (former boiler room) at 368 Congress Street, Boston: An electrical room within the old bolier room, in which 480 volt equipment with live parts were located, did not have a sign on door indicating such room should only be entered by qualified persons.

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: (a) Location - 1st floor switchgear room (former boiler room) at 368 Congress Street, Boston: On or about 06/28/12, the side of a live 120/208 vac electrical panel had several 1/2" romex connectors unused openings. (b) Location - 1st floor switchgear room (former boiler room) at 368 Congress Street, Boston: On or about 06/28/12, the 120/208 vac electrical panel had three breaker openings taped up, ineffectively blanked, to protect from live parts.
