29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) in that, employees performing maintenance on a sign at 3075 North Rocky Point Drive East in Tampa (FL), were not protected from falls at heights less than 16.0 feet. Personal fall protection provided and used on site was not sufficient to protect employees from a fall in that, 6 feet lanyards were used to work on the vehicle-mounted aerial lift. Hazard observed on or about 07/10/2012. FASTPOST OF TAMPA BAY WAS PREVIOUSLY CITED FOR VIOLATION OF THE OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.67(C)(2)(V) WHICH WAS CONTAINED IN OSHA INSPECTION 316452283, CITATION NUMBER 01, ITEM NUMBER 01, ISSUED ON 04/24/2012, AND BECAME A FINAL ORDER ON 05/18/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 2230 NORTH DALE MABRY HIGHWAY, TAMPA, FL 33607.
