29 CFR 1910.304(g)(4)(i): The equipment grounding conductor and the grounding electrode conductor were not connected to the grounded circuit conductor on the supply side of the service disconnecting means, or on the supply side of the system disconnecting means or overcurrent devices when the system was separately derived. M/C Resolution, Stern Area: On or about 11 July 2012 and times prior thereto, a 120 volt Americ Air Blower had a missing ground pin. Ship was in Dry-dock for repairs and the blowers were connected to shore power. This exposed employees to an electrical shock hazard.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: M/C Resolution, Stern Area: On or about 11 July 2012 and times prior thereto, a 120 volt Americ Air Blower had exposed wires that were partially taped with black electrician taoe. Ship was in Drydock for repairs and the blowers were connected to shore power. This exposed employees to an electrical shock hazard.
