29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment:  Marine Railway, Marine Railway:  On or about April 8, 2015 and at times prior thereto, during hotwork, the employer could not provide a documented occupational hazard assessment.  This condition limited employee information to hazards in their workplace and the proper selection of personal protective equipment.

29 CFR 1915.502(a): A written fire safety plan had not been developed and implemented by the employer that covered all of the actions necessary to ensure the employees safety in the event of a fire:  Marine Railway, Marine Railway:  On or about April 8, 2015 and at times prior thereto, during hotwork, the employer could not provide a written fire safety plan.  This condition limited employee and contractor information on fire hazards in their workplace and the proper application of fire watch duties.
