29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.        RJ Paquette, LLC, Holden, MA; Storerooms and bag house area were not kept clean and orderly.  Equipment and surfaces in the bag house were covered with cement dust that if allowed to get wet, became caustic and would damage electrical insulation.  Storerooms were not kept orderly creating fire, tripping, and falling hazards for employees walking in or through the area.

29 CFR 1910.146(c)(3):     When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit spaces:    Cement silo, RJ Paquette, Holden, MA; The employer did not take effective measures to prevent an employee from entering the cement silo, a permit required confined space, on July 12, 2012 resulting in an engulfment accident sufferred by the employee.
