RJ Paquette, LLC, Holden, MA; Storerooms and bag house area were not kept clean and orderly. Equipment and surfaces in the bag house were covered with cement dust that if allowed to get wet, became caustic and would damage electrical insulation. Storerooms were not kept orderly creating fire, tripping, and falling hazards for employees walking in or through the area.
