29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced: Dry Sack room: On or about July 12, 2012 and times prior thereto, the welding stinger was cut to the conductor at 24 inches from the holder. This exposed employees to electric shock hazards from coming in contact with the conductor during welding operations.

29 CFR 1910.37(a)(4): Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) must be in proper working order at all times. 2nd floor living area: On or about July 12, 2012 and times prior thereto, both exit signs lights for both exit access areas were not working. This exposed employees to smoke inhalation hazards from delays in exiting the area. Dry Sack Room: On or about 12 July 2012 and times prior thereto, the emergency lighting in the Dry Sack Room was inoperable. This exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.
