29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) Construction Site: On or about July 16, 2012, and at times prior thereto, employees were exposed to dermal hazards while installing tile and handling mortar without rubber gloves.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection: (a) Construction Site: On or about July 16, 2012, and at times prior thereto, an employee was exposed to electrical shock hazards while operating a Makita rotary hammer which was provided power with a cord plugged into an outlet without a ground fault circuit interrupter.
