29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables running through doorways, windows, or similar openings were not protected from damage. Sharp corners and projections were not avoided. Flexible cords and cables may only pass through doorways or other pinch points, if protection is provided to avoid damage. Construction Project at Building # 22206 at Anderson Air Force Based, Guam: On or about July 17, 2012 and at times prior thereto, a flexible cord plugged in the project office to provide power in the work area was run through a doorway jam and not protected from damage from the closing door.

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: Renovation project at Building 222026, AAFB, Guam: On or about July 17, 2012, and at times prior thereto, the controlling employer P & S Construction failed to ensure that its sub-contractor, Set Pacific, protected its employees from the hazard of electric shock while performing renovation and maintenance electrical work duties on and around two power panels that had openings allowing accidental contact to the energized conductors and exposing employees to the hazard of electric shock.
