OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of being struck by sharp pieces of PVC:    On or about June 22, 2012:    Employees were exposed to struck-by hazards during the use of compressed air to run tools and inflate tires when unprotected PVC piping was used to transport compressed air (110 PSI) during working operations.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to:    Only use material with shatter resistance properties. (Reference Plastic Pipe Institute)

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about June 22, 2012:    Employees were exposed to eye injuries when using corrosive chemicals to include Luma Brite a cleaner containing hydrofluoric acid and Hotsy Brown a caustic based cleaner without having an emergency eyewash station within the work area.

29 CFR 1910.177(d)(2):     The employer did not provide a restraining device or a barrier to be used when inflating tires on single piece rim wheels which are not bolted onto the vehicles:    On or about June 22, 2012:    Employees were exposed to struck by hazards when the employer failed to provide a restraing device, or have policy in place that servicing single piece rims could only be performed while bolted to the vehicle.
