29 CFR 1926.304(d): When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position. On or about July 16, 2012, on the east side of the construction site, employees engaged in framing activities utilized two skill saws with the lower guard secured in place by a wooden block to prevent the guard from covering the blade while cutting material exposing employees to the hazard of amputations.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about July 16, 2012, on the east side of the construction site, employees engaged in framing activities utilized an extension cord without a strain relief device to operate a skill saw exposing employees to an electrical shock hazard.
