Condition exposed employees to the hazard of projectiles to the eyes.

Condition exposed employees to a fall hazard of about 10 feet.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b). Note: If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502: a) El Dorado Construction at 1303 Sumac Avenue, Boulder, CO 80304: On or before July 14, 2012, the employer did not ensure that employees were using fall protection when shingling a two-story residence with a 5 in 12 pitch. Condition exposed employees to fall hazards of up to 20 feet. Alex Carrillo (Alejandro Carillo), the owner of El Dorado Construction, was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 314660374, citation number 1, item 1, issued November 2, 2010 and became a final order on January 22, 2011, with respect to a workplace located at 11853 Hannibal Street, Commerce City, CO 80022. Alejandro currently operates under the trade name El Dorado Construction.
