29 CFR 1910.95(c)(1):     The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) On or about 7/31/12 - The employee operating the stacker/folder machine was exposed to occuaptional noise at 97.06% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 89.7 dBA. The sampling was performed for 444 minutes on 7/31/12. Zero exposure is assumed for the remainder of the shift.    a) On or about 7/31/12 - The employee operating the stacker/folder machine was exposed to occuaptional noise at 82.77% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 88.6 dBA. The sampling was performed for 444 minutes on 7/31/12. Zero exposure is assumed for the remainder of the shift.
