29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    a)  Shop area, 16567 Lincoln Highway, Breezewood, PA 15533: Observed on or about July 12, 2012, a work platform used with a forklift to lift personnel was not approved by the manufacturer.  Employees using the work platform to perform building maintenance or to retrieve tires from the upper level of a tire rack were exposed to a fall hazard of approximately 8 feet to the concrete floor below.

Observed on or about July 12, 2012, unsecured cylinders of acetylene and oxygen were observed in the cylinder storage area.  Employees working in and near Bay One were exposed to struck-by hazards created by the unsecured cylinders.
