29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: Site, south end, the personal fall arrest system provided to a welder working at a height of approximately 16 feet was rigged in such a maanner to allow contact with the concrete lower level, exposing the employee to a fall hazard.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval, and the capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly. At the site, the JLG Industries Sky Trak all terrain forklift Model 6042, Serial Number 0160025641, was modified by underslinging steel vertical columns from a tyne, exposing employees to crush and struck by hazards.

29 CFR 1926.602(c)(1)(viii): A lift truck, equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, did not have the following precautions to include (A) Use of a safety platform firmly secured to the lifting carriage and/or forks (B) Means whereby personnel on the platform can shut off power to the truck (C) Protection from falling objects as indicated necessary by the operating conditions. At the site: JLG industries Sky Trak all terrain forklift, model 6042, serial number 0160025641, was used to elevate an employee to a height of approximately 18 feet while standing on the tynes of the all terrain forklift to set a vertical steel column, exposing the employee to a fall hazard.
