All ladders were not inspected regularly, with the intervals between inspections being determined by use and exposure. On or about 7/16/12 at 2720 Candler Highway Gainesville, GA; employees were exposed to potential fall hazards of 5' where the fixed ladder that is attached to the concrete silo was bent from vehicle contact.
