OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) 321 East Broadway, Bridger MT, 59014: On or about July 17, 2012 and at times prior thereto, employees were exposed to falling distances of approximately 40 feet down elevator shafts due to the lack of guardrails on the Unidentified Hand Powered Elevator.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or over current device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: (a) 321 East Broadway, Bridger MT, 59014: On or about July 17, 2012 and at times prior thereto, employees were exposed to shock hazards if circuits needed disconnected in the event of an emergency and/or amputation hazards if the wrong breakers were opened while equipment was locked and tagged out for maintenance because the breakers were not identified.
