29 CFR 1926.95(a):   Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) At the site: On July 16, 2012 during spray and brush painting, the employer did not provide hand protection from chemical absorption hazards present including but not limited to: ethylene glycol ethers, acetic acid, alkyl esters and acetates.     b) At the site: On July 16, 2012 during spray and brush painting, the employer did not provide eye protection from chemical hazards present including but not limited to: ethylene glycol ethers, acetic acid, alkyl esters and acetates.

29 CFR 1926.453(b)(2)(v):   A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:     At the site in the aerial lift, JLG Industries, Model 600 S, Serial Number Unknown: On July 16, 2012 the employer did not ensure an employee conducting painting from the aerial lift was attached by lanyard to the boom or basket, exposing employees to a fall hazard.
