1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On the north side of an excavation, employees were installing a 36 inch water main inside an irregular excavation that meaured 14.5 feet deep at its deepest point by 120 feet long by 25 feet wide. The exposed employee was not protected from a cave-in at the open end of the trench box at the north end of the excavation.

On the north side of an excavation, employees were installing a 36 inch water main inside an irregular excavation that meaured 14.5 feet deep at its deepest point by 120 feet long by 25 feet wide. A trench box was inproperly positioned, exceeding 2 feet from the bottom of the trench box to the excavation floor.
