29 CFR 1926.502(d)(16)(iii):   Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    At the site located at 9550 Seawall Boulevard, Galveston, TX:  On July 13, 2012 the employer did not ensure the personal fall arrest system being used by a framer walking on the top sill of the second level exterior wall was rigged to ensure the employee could not free fall more than 6 feet or contact any lower level, exposing employees to blunt force trauma hazards.

29 CFR 1926.1053(b)(1):   Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At the site located at 9550 Seawall Boulevard, Galveston, TX:  On July 13, 2012 the employer did not ensure the side rails of the ladder being used to access the second story wall top sill extended at least 3 feet above the top sill.
