At the site located at 9550 Seawall Boulevard, Galveston, TX:  On July 13, 2012 the employer did not ensure an employee working on a wall opening with a bottom edge approximately 9.25 feet above the exterior lower level was protected by the proper use of personal fall arrest system, exposing employees to fall hazards.
