On or about July 18, 2012, the employer failed to train and ensure that workers were protected against the hazards associated with the use and mixing of cement products.  Employees were not trained on the necessary personal protective equipment and health and safety hazards associated with working with cement products including Hanson Permanente, JIS R 5210 cement.
