29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (Construction Reference: 1926.59) (a) At the Redhorse Student Barracks construction worksite at Anderson Air Force Base, Guam. On or about July 18, 2012, the employer failed to train and ensure that workers were protected against the hazards associated with the use and mixing of cement products. Employees were not trained on the necessary personal protective equipment and health and safety hazards associated with working with cement products including Hanson Permanente, JIS R 5210 cement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) At the Redhorse Student Barracks construction worksite at Anderson Air Force Base, Guam. On or about July 18, 2012, a straight portable ladder used to access the building's roof did not extend at least 3 feet above the landing surface exposing workers to serious injury or fatality from falls.
