29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Area 2: On or about May 22, 2013, and at times prior thereto, the outer insulation used on the Skilsaw 15 amp 7 1/4 - Model SHD77 S/N 106528518 120 V 60 Hz @ 4600 per minute, was damaged and pulled away from the plug creating a hazard to employees using the saw. Employee was using saw at time of inspection.
