29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Chrysler Group, LLC - Perrysburg, Ohio: On or about June 27, 2012, the employer failed to assure that employees working on energized equipment were provided with, and used, protective equipment for the hands, face, head, and body.  Employees were exposed to electrical shock, arc flash/blast, and/or electrocution while removing florescent light fixtures and installing upgraded lighting fixtures on the second floor of the Administration building.    PER ISA AVD amended to read as such: On or about June 27, 2012, the employer failed to assure that employees working on energized equipment were provided with, and used, protective equipment for the hands, face, head, and body.  Employees were exposed to electrical shock and/or electrocution while removing florescent light fixtures and installing upgraded lighting fixtures on the second floor of the Administration building.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
