29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about 07/17/2011, at the job site - employees were permitted to work from a scaffold that was not equipped with guardrails, exposing the employees to 13'5" and 12' fall hazards.
