29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about July 13, 2012, at the Southwest corner of the construction site; an employee was working from a Snorkel Aerial Lift Model TB-60DZ Serial Number 56625 that was elevated to approximately 16 feet without fall protection. This condition exposed the employee to fall hazards.
