At the Raccoon International Golf Course located in Granville, Ohio an employee suffered a debilitating injury when a Little Giant portable step ladder was placed on the bed-liner in the back of a pick-up truck.  The ladder over-turned while the employee was trimming a tree.

29 CFR 1910.132(d)(2):     The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a. At the Raccoon International Golf Course located in Granville, Ohio the employer had Grounds Keepers that performed job tasks that should have required PPE such as safety glasses, face shields, head protection, but the employer had not completed a written hazard assessment.

At the Raccoon International Golf Course located in Granville, Ohio the employer had employees that operated power tools that created flying debris while performing grounds keeper duties, and did not require them to minimize the hazards with the use of safety glasses and face shields.
