29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) 1764 Pulaski Hwy, Bear, DE  Employees were using a 100 feet energized (120 volts) temporary flexible cord which had a missing ground pin, exposing the employees to an electrical hazard. The employees were using the flexible cord to energize a sawzall to cut penetrations into the roof for drops that would tie into the air conditioning unit. This violation was observed on or about June 25, 2012.
