29 CFR 1910.106(e)(6)(i): "General." Adequate precautions shall be taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat. a) On or about 7/17/12 in the waste liquid disposal area; waste solvent and type wash ( flashpoint 45 F ) from the presses are collected in a plastic pail. The contents of the pail are dumped into a plastic drain funnel on a 55 gallon metal waste drum. The metal drum is not grounded and there is no provision for bonding container(s) being poured into the metal waste drum. Abatement Certification

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a) On or about 7/17/12 in the Bindery area; a relocatable power strip was being used to supply power to the laminator ( model Accu III 2760 - serial number 27-26-026 12- volts 640 watts ) , static bar, and cutter. b) On or about 7/17/12 in the Bindery area; dual outlet metal electrical boxes with knockouts were being used for drop boxes to supply power to the bindery equipment.
