29 CFR 1926.451(c)(2)(i):  Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:      a)  Worksite, Exterior - Employee was installing siding and exterior window trim while working on a single scaffolding plank, approximately 12ft above the ground.  The plank was set up on the roof of the home; on or about 07/18/12.          Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The employee was walking on the roof and was exposed to a fall of approximately 12ft to the ground below; on or about 07/18/12.      Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
