29 CFR 1926.20(b)(1):     The employer did not initiate and maintain such programs as may be necessary to comply with this part:    An employee conducting painting work while sitting on the edge of a flat roof which was over 6 feet from a lower level were exposed to fall hazards, and the employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with the OSHA regulations.
