29 CFR 1910.1048(d)(1)(i):  Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde:    a)  On 09/18/2012, Painter employees working in a paint booth in the Touch-up area were not monitored to determine their exposure to formaldehyde when using a paint which contained formaldehyde.    NO ABATEMENT CERTIFICATION OR ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.1048(m)(1)(ii):  The employer did not communicate the hazards associated with formaldehyde in the workplace when formaldehyde gas, all mixtures or solutions composed of greater than 0.1 percent formaldehyde, and materials capable of releasing formaldehyde into the air, under reasonably foreseeable conditions of use, at concentrations reaching or exceeding 0.1 ppm. As a minimum, specific health hazards that the employer did not address were: cancer, irritation and sensitization of the skin and respiratory system, eye and throat irritation, and acute toxicity:    a)  On 09/18/2012, the employer did not communicate to Painter employees working with paint containing formaldehyde of the specific health hazards including cancer, irritation and sensitization of the skin and respiratory system, eye and throat irritation, and acute toxicity when employees worked in areas where concentrations of airborne formaldehyde reached or exceeded 0.1 ppm.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.1048(m)(5):  The employer did not develop, implement, and maintain at the workplace, a written hazard communication program for formaldehyde exposures in the workplace, which at a minimum described how the requirements specified in this paragraph for labels and other forms of warning and material safety data sheets, and paragraph (n) for employee information and training, would be met:    a)  On 09/18/2012, the employer did not develop or implement a written hazard communication program for formaldehyde exposures when Painter employees worked with paint containing formaldehyde in areas where concentrations of airborne formaldehyde reached or exceeded 0.1 ppm.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.

29 CFR 1910.134(h)(2)(i):  Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve:    On 07/18/2012, respirators were not stored to protect them from damage, contamination, dust and damaging chemicals:    a)  A half-face tight-fitting respirator located in a paint booth in the Touch-up department was not protected from paints and chemicals used in the paint booth.    b)  A half-face tight-fitting respirator located on a cabinet in the Commercial department was not protected from damage or dust.    ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED.
