29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about 05/14/2012, employees working 23' 6" above ground level on a walking/working surface and on the ground connecting/disconnecting brick pallets to a below-the-hook lifting device, the Balanced Pallet Lifter as described in ASME B30.20-2003. Employee(s) were not provided protective helmets to wear while attaching/disconnecting pallets of bricks to/from the Balanced Pallet Lifter.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 05/14/2012, at jobsite at 2857 Cherokee, St Louis, MO, employee engaged in demolition activities attaching pallets of brick to a Below-the-hook lifting device, the Balanced Pallet Lifter, 23' 6" above ground on the third floor of a 3-story building and was not provided fall protection.
