29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached or was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  On or about July 16, 2012 where two employees with personal fall arrest equipment were installing sheet metal decking on the roof of the new H.E.B. near Lakeline Mall and both employees were attached to a single 3M adjustable anchor model #4000, which was not installed in accordance with the manufacturers recommendations, exposing employees to a fall hazard. The 3M adjustable anchor model #4000 is designed to be used as an anchor point for no more than one person at a time and mounted with a minimum of 10 #8, 3/4 inch long sheet metal screws per side on raised rib of roof panel with pull on the long axis bracket.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that anchorage used for attachment for personal fall arrest equipment is capable of supporting at least 5,000 pounds (22.2 kN) per employee attached or is designed, installed and used as an anchor point for no more than one person at a time and mounted with a minimum of 10 #8, 3/4 inch long sheet metal screws per side on raised rib of roof panel with pull on the long axis bracket.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.  On or about July 16, 2012 an employee was installing sheet metal decking on the roof of the building under construction. The employee was wearing personal fall arrest system, but the rope grab was not adjusted to limit the free fall distance to 6 feet, exposing the employee to the hazard of falling and striking the ground.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee wearing personal fall arrest systems, when stopping a fall, will be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  On or about July 16, 2012 employees were installing sheet metal decking on the roof of the new H.E.B. store near Lakeline Mall up to approximately 21 feet high without receiving fall protection training from the employer, exposing the employee to a fall hazard.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that enables each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.
