29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked: A. On or about 07/10/12, scaffolds at 75-75 182nd Street. Flushing, NY Employees working on south side of the fabricated frame scaffolds were exposed to the hazard of falling approximately 13 ft. to the ground. The working platforms were installed with only two scaffold planks. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: A). On or about 07/10/12, at 75-75 182nd Street. Flushing, NY Employees were ascending/descending scaffolds by climbing the frame and were in danger of falling approximately 13 ft. to the ground. An access ladder or equivalent safe devices were not provided. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

Flushing, NY Employees applying stucco around the windows/house on the scaffold south side of the house were exposed to the hazard of falling approximately 13 ft. to the ground, respectively. Guardrails or personal fall arrest systems were not provided. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
