29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Location - Russel Street, Peabody, MA:  On or about 7/19/12, employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    (a) Location - Russel Street, Peabody, MA:  On or about 7/19/12, employees were exposed to fall hazards while working on a roof without fall protection.    Cory Rousseau Roofing was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926. 501 (b) (13) which was contained in OSHA Inspection Number 314731571, Citation Number 01, Item Number 1 a Issued 9/3/10 with respect to workplace located at 588 Bay Road, Hamilton , MA 01982. The final order date for the citation was on 10/11/10.
