29 CFR 1910.22(a)(1):     All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On or about 07/10/12 employees were exposed to a tripping hazard due to the employer keeping the storage garage on the west side in disarray.  Boxes, trash, parts and other items were thrown all over the garage area.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    On or about 07/10/12 an employee was exposed to an electrical hazard while operating the breaker for the light switch in the electrical panel on the east side of the warehouse.  The panel had an unused opening.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(4)(i):     Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely    On or about 07/10/12 employees were exposed to a struck by hazard while operating a power industrial truck to retrieve and store boxes in the garage, fill the line and drop pallets in the production department of the warehouse.  The employer did not provide retraining for the employees to ensure that they had the ability to safely operate a power industrial truck.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
