In the warehouse, on July 17, 2012, and at times prior thereto; employees were operating a Doosan forklift model G25E serial number 5778828 on the warehouse floor and was not wearing a seatbelt, exposing himself to being crushed if forklift turns over.     b. In the warehouse, on July 17, 2012, and at times prior thereto; employees were operating a Cat forklift model PD 11000 Serial number AT28C50317 on the warehouse floor and was not wearing a seatbelt, exposing himself to being crushed if forklift turns over.
