29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a.     On the load out platform where employees enter a runway to load rail cars, the end of the platform was not equipped with a fall protection system at the end where employees reach for a retactable life line thereby exposing the employees to a fall hazard.

29 CFR 1910.132(a):     Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a.     On the loading platform where employees connected to a horizontal lifeine when loading or unloading rail cars, the employer did not perform a hazard assessment to determine if tyhe fall protection system wasin proper working order thereby exposing employees to a fall hazard.    ANSI/ASSE Z359.1 Safety Requirements for Personal Fall Arrest Systems, Subsystems and Componets indicates fall protection systems must be inspected by the user before each use and additionally by a competent person other than the user at intervals of no more than one year.    ANSI/ASSE Z359.1 Section 7.1 requires prior to selecting a Personal Fall Arrest System, a competent person shall make an assessment of the workplace condiions where the equipment is required and understand the intended use of such equipment.

29 CFR 1910.219(d)(1):     Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    a.     On the Lake pit for Drag 1 and Drag 2, the belt and pulley was not completely enclosed on the back of the guard thereby exposing employees to a caught-in hazard.     b.      On the motor to Bin 7 drag where corn is moved from the bin and to the bin durin loading and unloading of product, the back side of the belt and pulley was not guarded tereby exposing employees to a caught-in hazard.    c.       On the motor to Bin 6 drag conveyor where product is moved throughout the facility for loading and unloading, the backside of the guard was missing to guard the belt ad pulley thereby exposing employees to a caught-in hazard.     The Coshocton Grain Company was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.219(d)(1) which was contained in OSHA inspection number 63109 , Citation Number 1, Item 8  and was affirmed as a final order on August 17, 2011, with respect to a workplace located at 475 Browns Lane, Coshocton, Ohio 43812.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    a.     In the electrical room on Panel DP-5, there were five missing circuit breakers on the right side of the energized 480 volt electrical panel thereby exposing employees to a contact hazard.    The Coshocton Grain Company was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 63109, Citation 1, Item number 10(b) and was affirmed as a final order on August 17, 2011, with respect to a workplace located at 475 Brown Lane, Coshocton, Ohio 43812.
