29 CFR 1910.119(h)(3)(iii): The contract employer did not document that each contract employee has received and understood the traininig required by this paragraph. On or about 16 July 2012, and at times prior thereto, the employer did not ensure that training in the work practices necessary to safely perform his/her job was documented for contract employees that were maintaining the ammonia system for the SCR/DeNOx unit. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that training performed on safe work practices is documented for employees maintaining the ammonia system.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation. At the inspection site, for calendar year 2009, incomplete information for case# 09-01 on the Log materially impaired understandability of the nature of hazards, injuries and illnesses in the workplace in that the object/substance that directly injured or made the person ill was not recorded. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure entries in column F of the OSHA 300 Log provide enough description to indicate the injury or illness, the parts of the body affected, and the object/substance that directly injured or made the person ill.
