29 CFR 1910.303(f)(2): Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. At the job site, 717 Bertrand, St Marys, Kansas, employees were operating disconnect breakers that were not marked to indicate their purpose. Employees were exposed to electric shock hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. The circuit breaker cabinet near the feed warehouse south door had unused openings in the box in the panel and at the bottom of the cabinet. Employees were exposed to electric shock hazards from unused openings. (1). Bottom of the circuit breaker box had a hole opening. (2). Circuit breaker inside panel had breaker hole opening.
