29 CFR 1926.760(b)(1): Each connector was not protected in accordance with paragraphe (a)(1) of this section from fall hazards of more than two stories or 30 feet (9.1 m) above a lower level, whichever is less. Steel structure ridge, east side: An employee was exposed to a fall hazard of 30-41 feet while working above the ground level from a steel structure without adequate fall protection. The employees were connecting and erecting steel at the ceiling of the building.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: Documentation & Training: The employer failed to provide fall protection training to two employees who were exposed to fall hazards of 30-41 feet. An employee was observed connecting steel to the ceiling of a steel structure and had not received prior training on fall hazard awareness.
