29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      As of 07/13/2012 the employer failed to establish an effective program for the control of hazardous energy at the facility.  Employees complete service and maintenance on equipment such as, but not limited to, slitter machinery, the cross-over(conveyor) unit, the facer machinery and the friction strip welder.

Employees tasked with making adjustments (to various gauge) coiled metal, so that it can be thread through the straightener, and then ran through the knives, are not protected from to hazards created bypoint of operation, ingoing nip points, rotating parts (etc.).
