29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  At the work site on July 19, 2012, employees were exposed to fall hazards while working from the roof of a commercial building without fall protection approximately 10 feet above the lower surface.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) At the work site and on July 19, 2012, employees used a 24 foot extension ladder with side rails that extended approximately 2 inches above the upper landing surface to access the roof.
